The South West CCAC works as a team with family physicians and nurse practitioners. They can help your patients live safely and comfortably at home, and make the transition from hospital to home, and from home to other living arrangements.

South West CCAC Symptom Response Kit - this kit includes medications that can be ordered by a physician, to be available in a client’s home to relieve potential symptoms for patients requiring hospice palliative care services or who are at the end of life stage in their disease management.
