Breast Cancer Follow-up Care

Pink Notes for Primary Care artwork

The South West Regional Cancer Program has developed a tool to help you care for breast cancer patients post treatment. “Pink Notes for Primary Care” is comprised of evidence-based guidelines for breast cancer patient follow up and side effects.

The innovative (and colourful) guide is part of a toolkit that was created in response to an initiative launched by Cancer Care Ontario last year aimed at facilitating an earlier discharge of “well” breast cancer patients from tertiary care centres back to primary care providers in the region.

Pink Notes for Primary Care includes:

  • Surveillance Guidelines
  • Surveillance Imaging
  • Lymphedema
  • Fatigue/Dyspnea
  • Weight Gain
  • Menopausal Symptoms
  • Peripheral Neuopathy/Arthralgia
  • Return of Menses While on Endocrine Suppression Therapy
  • Skin Changes in the Treatment Field
  • Low Bone Minneral Density/Osteoporosis
  • Psychosocial Concerns
